Our Social Prescriber Link Workers supports people with a wide range of social, emotional or practical needs, and are focused on improving mental health and physical wellbeing and seeks to address people’s needs in a holistic way. They aim to support individuals to take greater control of their own health.

Our First Contact Physiotherapist (FCP), is a healthcare professional working at above GP surgeries, and being the ‘first contact’ for a patient. This means that you do not need to see a GP in order to see the FCP, if you have a musculoskeletal (MSK) problem. The FCP can undertake specialist assessment and provide appropriate advice and establish a management plan that is appropriate for you. Physiotherapists working as FCP’s are experienced musculoskeletal clinicians who have experience of assessing and managing bone, muscle, nerve or joint conditions.
Supporting Staff
- Practice Care Co-ordinators - to tackle health inequalities among vulnerable groups and increase the uptake in attendance to physical health checks and health and well being screenings.
- Nurse Associate - Nursing associate is a new role within the nursing team. Nursing associates work with healthcare support workers and registered nurses to deliver care for patients.
- PCN IT & Digital Lead - The key objective of the role is to support PCNs and their practices to deliver ongoing improvement to services. Improvement projects are not restricted to those that focus on the utilisation of digital tools and could include any transformation work that utilises data or quality improvement tools to improve quality, drive efficiency or improve patient experience
- GP Assistant - General Practice Assistants provide a support role, carrying out administrative tasks, combined in some areas with basic clinical duties. They can help to free up GPs time and contribute to the smooth running of appointments, improving patients experience in the surgery
- Diabetic Specialist Nurse - Providing enhanced diabetes care for patient with complex diabetes and needs, who require the assessment and advice from a specialist nurse. This service is provided during our PCN Enhanced Access Clinics to support patients to safely manage their condition.
